New Patients

Financial Information

We accept cash, checks, and all major credit cards. If you need to cancel or reschedule your appointment, we ask that you contact our dental practice at least 24 hours prior to your visit to avoid a cancellation fee.

Insurance

We accept all dental insurances and as a courtesy we file claims with your insurance companies as an added convenience to you.

For our patients without insurance coverage, we accept CareCredit a national, third-party financing company that offers low- and no-interest payment plans to help you finance your dental care. You may apply online at the CareCredit website or at our office.

Patient Forms

Before your visit, fill out the forms below and bring them with you to your appointment. Remember, if you are bringing your child to our office, you will need to complete the Minor Consent Form along with the other documents.
